How It Works
Becoming a member of our Patient Loyalty Plan is easy! If you don’t have dental insurance, all you have to do is contact Smile Exchange at the number above and inform us that you are interested in signing up or learning more about our Patient Loyalty Plan. We will then proceed to help you choose which plan is best for you and your family. Once you choose the option that’s right for you, you will be billed annually and have immediate access to your benefits.

Once you have successfully activated your membership, your Patient Loyalty Plan is valid at all Smile Exchange office locations.
Terms and Conditions
* This is only an example of the savings and treatment based on the Adult Complete plan. Needs are determined by a Smile Exchange dentist, price varies by location.

Terms and Conditions: The 100% satisfaction guarantee is applicable to the Patient Loyalty Plan only. Refunds must be issued within 30 days of activation and prior to starting treatment. For work not started: not valid for previous or ongoing work and cannot be combined with other discounts or dental discount programs. Discount taken off usual and customary fees for general dentistry services. Offer must be applied prior to completion of treatment and discount taken upon completion. Subscription payment must be received prior to the completion of any services beyond a new patient visit in the plan year. Annual renewal is required to remain in program and the guarantor assumes responsibility for annual payment for all family. Interruption of subscription during treatment will result in discounts being void and full fee applied to ongoing treatment.
